Please complete the fields that apply to you. However, you MUST complete the required fields which are marked with red *. The more information you provide us, the closer and the better we would be able to serve your needs. We strongly recommend that you provide us with as much information as you can. The *required fields are: 1. First Name • must not be empty • can be alphanumeric (eg. a to z, 0 to 9, or special characters such as "-") 2. Last Name • must not be empty • can be alphanumeric (eg. a to z, 0 to 9, or special characters such as "-") 3. Email • must be valid since we will notify you with the confirmation of your reservation • must contain at least an "@" and a "." 4. Check-In Date 5. Check-Out Date • both must be in "mm/dd/yyyy" format (eg. "12/14/2005" for December 14, 2005) • Check-In Date must be at least one day later than Today's Date, and Check-Out Date must be later than both Today's Date and Check-In Date. (eg. if Today's Date is 12/14/2005, Check-In Date can be any date starting from 12/15/2005 to at least one day before the Check-Out Date, Check-Out Date can be any date starting from 12/16/2005) 6. Room Type 1 • must be chosen • if you reserve more than one type of room, please choose Room Type 2 to 5
